Unless you specify otherwise on this form, the name on your diploma will be the name that appears on your official student record on file with the Office of the Registrar. If you are unsure how your name is recorded, you may check by confirming how it is displayed on NECIN. If you want the name that is on your official student record to appear on your diploma, select "Name is fine as is" below.
To use a different name on your diploma than the name that appears in the Office of the Registrar's file, you must complete and submit the Name Change Form and select "I want to change my name on my diploma" below. Name changes require that legal documentation be submitted to the Office of the Registrar.
If you want a variation of your official name to appear on your diploma, select "I want to change my name on my diploma" below. Acceptable variations include adding diacritical marks (example: á) or changing a name to an initial or an initial to a name. Examples of acceptable options appear below. These types of variations do not require any legal documentation.
